A D4 medical officially known as the D4 Medical Examination Report is a mandatory health and fitness assessment required by the DVLA for all Group 2 licence holders. Group 2 licences cover vehicles over 3.5 tonnes and passenger-carrying vehicles, including lorries, HGVs, LGVs, buses, coaches, ambulances and certain minibuses. The examination must be completed by a GMC-registered doctor, and the completed D4 form submitted to the DVLA as part of your licence application or renewal. You may also hear it called an HGV medical, LGV medical, lorry driver medical or Group 2 medical they all refer to the same DVLA-required assessment.

The DVLA requires HGV and LGV drivers to pass D4 medicals at key points throughout their career. The same schedule applies to PCV, C1 and D1 licence holders.
Missing a renewal medical can cause your licence to lapse, meaning you cannot legally drive professionally until a new medical is passed and submitted to the DVLA. We recommend booking your D4 medical at least 6 weeks before your licence expiry date to allow time for DVLA processing.
Your completed D4 form is valid for 4 months from the date signed by the examining doctor.
You must submit it to the DVLA within this window if you miss the deadline, a new examination and a new form will be required.
